ASSUMPTION OF RISKS and publicity waiver

DISCLAIMER CLAUSE

Slater Dykema, operating as Platinum Star Power Skating, its employees, volunteers and representatives (“Platinum”) is not responsible for any injury, loss or damage of any kind sustained by any person while participating in the power skating instruction (“The Program”), including injury, loss or damage which might be caused by the negligence of Platinum.

ASSUMPTION OF RISKS

The Participant acknowledges that he/she is aware of the possible risks, dangers and hazards associated with his/her involvement in The Program, including the possible risk of severe or fatal injury to The Participant.

COVID-19 RISKS

I understand that the risk of becoming exposed to or infected by COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Platinum Star employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at a Platinum Star program. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Platinum Star Power Skating, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Platinum Star Power Skating and its employees, whether a COVID-19 infection occurs before, during, or after participation in any of our on-ice or off-ice programs.

RELEASE OF LIABILITY

In return for Platinum allowing The Participant's participation in The Program and related activities, The Participant agrees to ACCEPT ALL RISKS arising out of, associated with or related to his/her involvement in The Program, even though such risks may have been caused by the negligence of Platinum.

PUBLICITY WAIVER

I grant permission for The Participant's name/photo to be published or publicly displayed on print and social media for the sole purpose of promoting Platinum.

ACKNOWLEDGEMENT

I acknowledge that I have read this waiver, and that by submitting this online Registration form, this waiver is binding.
